The CSEA Health Insurance Rebate Program will provide a rebate of up to $3,000 to assist with the purchasing cost of 2022 health coverage under the NY State of Health exchange. We can only reimburse you for the premium expenses you were required to pay out-of-pocket (after subsidies were applied). We will require at a minimum for final determination the following: * A copy of your Insurance Identification Card * The 2022 IRS Form 1095-A you received in January 2023 * A copy of your NY State of Health Marketplace plan enrollment letter You are eligible if you meet the following three conditions: * You have enrolled in the NY State of Health Benefit Exchange and pay premiums out-of-pocket for health insurance, AND * You live outside NYC and are a New York State Registered Family OR a Licensed Group Family OR an Enrolled Legally-Exempt child care provider. * You are not on Medicaid Applications are due by close of business on July 31, 2023.
